Request to Fundraise
This form must be completed by all coaches/managers that wish to fundraise for their SPWHL team. A new form must be filled out for each fundraising event. Once approved the treasurer of the league will notify your team contact and the event may proceed.
Team Disclaimer:

I acknowledge that I have read and understood the terms and conditions enclosed in the SPWHL Fundraising Procedures. I understand that my failure, or my teams failure to abide by the rules set out by the SPWHL may result in disciplinary action and/or suspension from league sponsored activities. I agree that no Fundraising Activities will be commenced without approval of this request. I agree to provide full details of funds raised and expenditures of funds (including receipts) in the Team’s Financial Report.
