Coach Evaluation Form 2024-25

The goal of this form is to help coaches and the Soo Pee Wee Hockey Association continue to grow and improve both individually and as a program. Your honest and thoughtful feedback is crucial in enhancing the overall quality of coaching. All responses will remain confidential. To ensure anonymity, each parent will be assigned a random 4-digit code to ensure only the parents of that team are submitting a evaluation. These codes are unique to each team. Parents are welcome to submit a separate form for each coach they wish to provide feedback on, including the head coach (who must be included in your submissions). If you are evaluating more than one coach, please complete and submit a new form for each individual coach. Thank you for your input and support in strengthening our hockey community.
